DAVENPORT, IOWA (April 28, 2021) — HyVee and Davenport Community School District have partnered to offer Pfizer COVID-19 vaccinations to DCSD students aged 16 years and older, with parental consent. Adult family members of DCSD students can also request and receive vaccinations.
Pfizer vaccination requires two doses. The first dose will be available on May 4 and 6, 4-7 pm. Second vaccine doses will be scheduled during the first-dose appointment. The clinic will be held at the former Gordman’s Store at 3860 Elmore Avenue in Davenport.
Consent forms for DCSD students and their families are available online:
https://www.hy-vee.com/my-pharmacy/covid-vaccine-consent.
A new consent form should be filled out for each 16- or 17-year-old receiving the vaccine, and a separate appointment must be scheduled for each family member who wants to be vaccinated.
According to DCSD policy, if a positive case of COVID-19 is discovered in a DCSD building, families of students who are considered to be in close contact with the person with a positive COVID-19 test will be contacted and provided with important instructions. Those families should then follow the instructions provided when contacted.
More COVID-19 status updates from the DCSD can be found at this webpage:
